Responses are combined and paraphrased rather than reproduced as direct quotes, reducing the risk that wording identifies a colleague.

Safeguarding
Clear human responsibility
A supervisor confirms that nothing needs further follow-up before the physician sees the report. Serious safety concerns follow a defined, consent-based process.
How feedback providers are protected
Identity deleted after sign-off: When the supervisor signs off the review, each feedback provider's name and email are permanently deleted from the record.
Consent before identity is shared: If a feedback provider raises a serious safety concern, Refract asks for their consent before sharing their identity.
